CPR/First Aid Certification (If you do not have current certification, please include documentation to verify that you are willing to obtain this certification through training we offer.)

Letter of Introduction

Letter(s) of Recommendation (Include 2 letters that are dated less than 2 years ago)
Proof of HS Graduation (Copy of HS Diploma or HS Transcripts with graduation date)

TB Clearance, Fingerprint Clearance and Board approval will be required if employment is offered.
