Requirements / Qualifications

High School Diploma

Immunizations: Tdap (within the last 10 years), MMR (2 within a lifetime)

• Driver's License Copy (Current Drivers License and Proof of liability inusrance)
• Proof of HS Graduation (Copy of High School Diploma or GED)
